A Clinical Study of Liver Abscess



Abstract
This study is based on clinical analysis of 65 patients with liver abscess admitted to the department of surgery, Hallym university, Hangang sacred heart hospital, from January 1983 to December 1992.
@ES The results are as follows:
@EN 1) In the etiologic classification, 46 cases(70.8%) were pyogenic, 13 cases were amebic and 6 cases were mixed type.
2) Peak age of incidence was the 6th (23.1%) and 7th (23.1%) decade and mal to female ratio was 1.2:1.
3) The most common symptoms and signs were abdominal pain(78.5%), tenderness(73.8%) and fever and chill(67.7%) in that order of frequency.
4) 13 cases(28.3%) of pyogenic liver abscess had hepatobiliary disease.
5) 47 cases(72.2%) of abscess were located in the right lobe, 9 cases(13.9%) in the left lobe, and 9 cases(13.9%) in both lobes.
6) The most common etiology was cryptogenic(60.0%), followed by pathology in hepatobiliary tract(29.2%).
7) Abnormal chest X-ray findings were seen n 35 cases(53.8%), and the most frequent abnormal finding was pleural effusion (71.4).
8) The liver function test showed increased alkaline phosphatase(56.9%) and hypoalbuminemia(41.5%).
9) In the bacteriologic study, we obtained 63.3% of positive cultures, and streptococcus was the most frequently cultured organism(32.3%).
10) Percutaneous drainage was performed in 25 cases(38.5%), operative drainage in 24 cases (36.9%) and left lateral segmentectomy in 4 cases(6.2%).
11) Postoperative complications were occured in 3 cases(12.0%) with percutaneous drainage and 19 cases(63.3%) with operative drainage.
12) Overall mortality rate was 9.2% and the most common cause of death was sepsis.
